Choose “Apply for Re-issue of Passport.”

Select the reason as “Change in Existing Personal Particulars.”

Enter your new address carefully—it must match the address on your supporting proof document.

You can fill the application online or download the PDF, fill it manually, and upload it back.

Upload a scanned copy of a valid address proof.

Accepted proofs include:

Aadhaar card
